
Are there any forms that I need to complete?
An intake packet will be emailed before your first session. Please take a moment to complete the necessary documents and upload an image of your insurance card.
Do you accept insurance?
We accept payments directly from insurance companies that we are in network with. It is the client's responsibility to pay for any copays/deductibles/coinsurance at the time of service. We are happy to assist with the necessary documents for those seeking reimbursement through their out of network benefits.
What are your fees?
The fee for individual and couples therapy sessions is $150
What forms of payment are accepted?
All major credit cards are accepted. It is the practice policy that clients maintain an active credit card on file.
What is your policy on missed appointments?
We ask for 24 hour notice to reschedule or cancel sessions. Less than 24 hour notice will result in the full session charge.
Do you provide medication management?
We are unable to prescribe medication but work closely with prescribers and can provide referrals upon request.
